Northern Human Services provides an array of opportunities for individuals through our Community Participation Services (CPS) and Community Support Services (CSS) programs.
Community Participation Services (also known as Day Services) support an individual’s participation in a variety of integrated community activities and settings, including vocational and volunteer opportunities.
Community Support Services assist individuals who have developed or are trying to develop skills to live independently within the community and include assistance and training to maintain and improve skills related to daily living such as shopping, banking, laundry, food preparation and budgeting.
Northern Human Services provides opportunities and support to assist individuals in finding meaningful work or volunteer opportunities, establishing and meeting physical fitness goals and/or social opportunities, and/or navigating or mastering independent living skills. Staff at Northern work with an individual and their family or guardian to understand which goals, hopes and interests are most important to them and, together as a team, develop an individual service plan that helps them live a meaningful life in their own community.
Person-Centered Planning
No two service programs are alike, as our caring staff seek to provide daily living activities that bring meaning and value to each individual. Northern Human Services incorporates a collaborative team approach and a people-first philosophy. Depending on an individual’s service plan, team members may include a service coordinator, team leader, community integrator, vocational coordinator, family member(s), and residential team members, such an enhanced family caregiver or a member of the residential staff team, or a family support coordinator.
Individual Service Plan
Living a meaningful life in the community means taking time to understand each individual we serve; learning about their interests, hopes, and goals. Individuals, with support from their family and the Northern Human Services team, will create a plan that may include:
- A job or volunteer experience.
- Opportunities to work on social, life, and physical fitness goals.
- Supports with independent living skills.
- Access to learning new activities of personal interest.
- Living arrangements, such as living with family, living in with an enhanced family care provider, or in a residential home that may provide need additional behavioral and physical supports.
Once a service plan and supporting budget is set, our staff works with families to provide staffing to support the plan.
