For more than 45 years, Northern Human Services has been at the forefront of community inclusion, services and supports for individuals living with a developmental disability or related disorders in the North Country of New Hampshire. Our service area includes Carroll, Coos and upper Grafton counties.

Developmental Services for Youth
Family Centered Early Supports and Services
Birth to 3 years
Family Centered Early Support & Services is for children from birth to age three who may have a developmental delay or disability; are at risk of becoming delayed because of environmental or medical reasons; and/or have an established condition, such as down syndrome or cerebral palsy. Anyone may make a referral for a child experiencing a delay.

Ages 3-22
In-Home Supports provide home and community-based services for children between the ages of 3 and 22 (or completion of high school) who have a developmental disability and significant medical or behavioral challenges. In-Home supports help children to remain living at home with their family and actively engaged with their community.

Ages 14-22
Transition services are provided to individuals with developmental/intellectual disabilities between the ages of 14 and 22 who are nearing the end of their formal education and will need support services after graduation. Transition coordinators work with individuals and their parents to ensure a smooth transition into adult services including future planning and budget development.

Being a parent of a child with a developmental disability poses unique challenges and sorting through information, resources and services, and figuring out how to pay for them is often confusing and overwhelming. Our Family Support Advisory program helps families understand and navigate important resources that can help families remain the primary source of support for an individual.
Learn moreDevelopmental Services for Adults
Our developmental services programs are designed to help individuals live a meaningful life in their community. Using a person-centered approach, we develop individual service plans that reflects their interests, hopes, and goals. The process is collaborative between the individual, their service coordinator, family members/guardian and staff. A individual service plan may include:
- A job or volunteer experience.
- Opportunities to work on social, life, and physical fitness goals.
- Supports with independent living skills.
- Access to learning new activities of personal interest.
- Living arrangements.
Service Coordination
Service Coordination is an important part of an individual service plan for individuals with a developmental disability. Your service coordinator helps you develop and receive the services in your individual service plan. Families have the choice to either work with a service coordinator through Northern Human Services and utilize our Self-Directed Services program or work with an external service coordinator and participate in our Community Support Services and/or Community Participation Service programs.
Community Participation & Support Services
Our Community Participation Services program provides opportunities for active involvement in the local community. With support and guidance from a direct support professional, individuals who participate in our community participation services program are empowered to work, volunteer, socialize and recreate in and around their local community. Community Support Services assists individuals who have the skills or are working towards developing the skills to live independently within their community.

Our Vocational Services/Supported Employment program assists adult individuals in becoming important members of their local community. Vocational coordinators and direct support professionals work with employers across the North Country to cultivate work environments accepting of diversity and provides individuals with the opportunity to learn life skills, develop greater self-esteem, and live a more interdependent life within their community.

Participant Directed & Managed Services (PDMS) allows you to control your care, set your own budget, and directly recruit, hire, and supervise your support staff. Families design, direct, implement and monitor all services. PDMS allows families to decide how much of their budget will go towards paying staff and other services; determine staffing schedule (days, evenings, weekends); and coordinate services.
Learn moreEnhanced Family Care/Home and Care Providing
Enhanced Family Care provides a family-like living environment with specialized support for individuals who choose to not live with a family member. Individuals are matched with certified Home Care Providers based on needs, interests, and preferences of the individual and the home care provider. HCPs welcome individuals with a developmental disability into their home, support their overall health and welfare, and help them develop skills including personal care, communication and social skills, medication administration, participation in household routines and involvement in community activities.

Individuals and family members who choose to live together automatically receive supports and services through our Family Support program. Family Support Coordinators tailor support services to the individual and their family according to their interests, preferences, competencies and needs. Collaboration with other service providers such as the individual’s school system, the local community, and the Family Support Advisory Council help families to get the most out of these services.
