Service Coordination is an important part of an individual service plan for individuals with a developmental disability.
Your service coordinator plays an important role in helping your receive the services as outlined in your individual service plan and will help you navigate/access other health, education and community needs.
Federal Medicaid regulations prohibit Northern Human Services from providing both service coordination and service program delivery. Families have the choice to either work with a service coordinator through Northern Human Services and utilize our Self-Directed Services program or work with an external service coordinator and participate in our Community Support Services and/or Community Participation Service programs.
Individuals/Families choosing Community Participation Services and/or Community Support Services
- Northern Human Services provides service coordination, assisting families in sourcing programming, and supports with other service providers.
- An external agency/organization provides service coordination and Northern Human Services is the service delivery provider.
Individuals/Families Choosing Self-Directed Services
- Northern Human Services provides services coordination, and the family seeks out service delivery providers with support from NHS. This option is for individuals/families who wish to self-direct the programs, services, staffing and supports. Northern Human Services provides “employer of record” services for direct support staff, while families manage budgeting, programming and other services.
