Transition services are one of our most frequently-used services. Transition services are offered to individuals with developmental/intellectual disabilities between the ages of 14 and 22 to ease the process from high school to adulthood.
Transition coordinators meet with individuals and their parents multiple time over the course of several years to ensure a smooth transition into adult services.
Transition coordinators work with individuals to develop future planning, using a person-centered approach that includes their interests and goals. Our coordinators collaborate with New Hampshire Department of Education Vocational Rehabilitation staff and school district personnel, and our vocational coordinators to help individuals build skills, explore interests to help individuals successfully transition from school to community services.
